To ensure a smooth start to your session, we have client intake and health history forms that need to be completed.

For your convenience, you can download, print, and fill out these forms beforehand and bring them to your appointment.​
If you're unable to complete them in advance, please arrive 5-10 minutes early to fill them out at our Cleveland office.

Late Arrivals/Missed Appointments/Cancellations:
We understand that unanticipated events happen occasionally in everyone’s life. In our desire to be effective and fair to all clients, advance notice is appreciated when canceling an appointment. This allows the opportunity for others to receive the benefits of massage therapy.
Late arrivals of 10 minutes or more will result in time deducted from your session at cost or the option to be rescheduled.

Cancellations:
Please text or call (216) 240-5919 at least 24hrs in advance and leave a voice message to cancel your appointment. Leave your name, phone number, and reschedule date & time. Your paid deposit (or session) will be transferred to another date or time.
Missed Appointments/No Call No Show:
Should you miss your scheduled session without prior notification (no call/no show, nc/ns), your initial deposit is non-refundable and non-transferable.
*1 missed appointment- reminder to call to cancel, notification of $25 fee that will be applied to next session.
*2 missed appointment- $25 non-refundable fee will be applied to scheduled session. Payable at the time of booking.
*3 missed appointments- Session must be paid in full at time of booking. Non-refundable but transferable to another date/time/person.
FORMS
Massage and medical history, emergency contact, medications, pain location, preferences & desired results from session.
An agreement to disclose medical information, what is provided by Resolutions, and acceptable client conduct.
Respect Policy

Before your session, please turn your cell phone off or place it on the silent setting so as to not disturb your session.
​
Any comments that are offensive, rude, derogatory, explicit or sexually suggestive actions, will result in immediate termination of session and client will still be responsible for full payment.

Your Comfort During Your Massage:
Please don't hesitate to let your therapist know immediately if you experience any pain or discomfort during your session. We are here to adjust the pressure, draping, or room environment to ensure you feel completely comfortable.

Hygiene:
For the comfort and well-being of both clients and therapists, we kindly request that you shower or bathe prior to your scheduled massage appointment.​

Children:
To ensure a tranquil environment for all our clients, we kindly request that children do not accompany you to your appointment unless they are the individual scheduled for a massage.
For clients under the age of 18 receiving a massage, a parent or legal guardian is required to be present during the entire service.
If a parent or guardian is unable to attend, we require a signed written note containing the following information:

Clear consent for the massage treatment.

We are committed to protecting your personal health information. All details you share are kept confidential in accordance with HIPAA regulations and will only be used to personalize your massage therapy sessions and help you achieve your wellness goals.

Disclosure of Medical Condition and Diagnosis:
To provide you with the safest and most beneficial massage, please openly communicate any existing medical conditions, diagnoses, and medications you are currently taking. It's also important to keep your therapist updated on any changes to your health or medications in the future. This information helps us select the most appropriate techniques for you.
